Where you live decides whether an emergency room is five minutes away or a highway drive. VA Black Hills runs two medical centers and a Rapid City outpatient clinic. Fort Meade sits immediately east of Sturgis and carries a 24/7 emergency department. Hot Springs carries dialysis and residential rehabilitation, with urgent care on weekday hours only.
Six towns on this site have hospital care inside them rather than down a highway. In winter that difference is bigger than the mileage looks.
Rapid City, Spearfish, Sturgis, Lead and Deadwood, Custer, Hot Springs
Cardiac and vascular care, the region's accredited stroke center, cancer care, pediatrics and a 28 bed NICU all sit in Rapid City. Sanford adds a second orthopedics and spine footprint there.
Spearfish and Sturgis each have a hospital with a 24/7 emergency department, and Spearfish is in the middle of an $80 million expansion of inpatient, ICU, surgical and specialty clinic space.
Custer runs the only hospital emergency department in the southwestern Hills between Custer and Hot Springs, and Hot Springs carries both an independent critical access hospital and a VA medical center.
The East Mall Drive emergency campus opened in Rapid City in June 2026. Spearfish is under construction. A separate Sanford inpatient medical center has been announced for Rapid City and is not open.

A 25 bed critical access hospital in Hot Springs with a 24 hour emergency room and a rural health clinic. Services include imaging, primary care, cardiac rehabilitation, rehabilitation, wound care and visiting specialty physicians in cardiology, orthopedics and podiatry.
Primary and secondary medical and surgical care for veterans, plus residential rehabilitation, extended nursing care and inpatient psychiatric services, delivered through the Fort Meade and Hot Springs medical centers and a set of outpatient and rural clinics. VA Black Hills holds sharing agreements with Ellsworth Air Force Base and the South Dakota Army National Guard. VA Black Hills reported a 5 star rating in the 2026 CMS Overall Hospital Quality Star Rating.
The practical difference between the three sites matters more than the system name. Fort Meade has a 24/7 emergency department. Hot Springs is a medical center with weekday urgent care hours and is not a 24 hour emergency room. Rapid City is an outpatient clinic.
Emergency care 24/7
The VA medical center on the Fort Meade campus immediately east of Sturgis, not inside Sturgis city limits. The facility is open 24/7 with a 24/7 emergency department, plus primary care and specialty services including mental health, PTSD and TBI care, spinal cord care, orthopedics, rehabilitation and urology.
Limited hours, no 24 hour emergency room
Urgent care runs Monday through Friday, 7:30am to 4pm, and is closed weekends and holidays. This campus is not a 24 hour emergency room.
A VA medical center on the Battle Mountain campus in Hot Springs carrying primary and specialty care, mental health, addiction and substance use treatment, dialysis, TBI care and residential rehabilitation programming. For a veteran who needs dialysis or residential treatment, this campus is the reason the southern Hills is on the list at all.

No emergency department
Outpatient VA care in Rapid City, including primary care. It is a weekday clinic rather than a hospital, so emergencies still go to a hospital emergency department.
One piece of context for veterans comparing systems: VA Black Hills reported a 5 star rating in the 2026 CMS Overall Hospital Quality Star Rating. Ratings move year to year, and eligibility, enrollment and referral questions belong with the VA rather than with a real estate page.

Rapid City Hospital is the Level II trauma center. Sanford's operating Black Hills footprint centers on Black Hills Surgical Hospital and Black Hills Orthopedic and Spine Center, which joined Sanford Health, along with additional Rapid City clinics and specialists. Sanford announced 32 new clinicians beginning practice in the Black Hills in 2026 across gastroenterology, dermatology, optometry, ophthalmology, family medicine and general surgery among other specialties.

A proposed 480,000 square foot campus next to Black Hills Orthopedic and Spine Center in Rapid City, planned to open with 168 inpatient beds and room to expand. Nothing here is available to a patient today, and plans of this size get revised.
Construction is scheduled to begin in 2027 and completion is expected by 2030, per Sanford's announcement.
